CLUSTER DE TURISMO MÉDICO DE PUEBLA AC (Puebla Care), with address at Av. Kepler 2143, Reserva Territorial Atlixcáyotl, 72190 Puebla, Pue., is responsible for the treatment of your personal data according to this Privacy Notice.

DATA COLLECTED

The personal contact and/or identification data that Puebla Care will process are name, age, gender, occupation, address, email, telephone (mobile and/or landline) and federal taxpayer registration.

SENSITIVE DATA

Puebla Care will collect and process sensitive data, related to health status, medical history and lifestyle information, necessary or convenient for the above mentioned purposes. Sensitive personal data will be kept and treated with strict security and confidentiality for purposes related to the provision of health services in accordance with this Privacy Notice.

 

PURPOSE OF THE PROCESSING OF PERSONAL DATA

Personal Data held by Puebla Care will be used for the following purposes:

 

1. Purposes necessary for the maintenance of the relationship with the data owner:

• Provision of medical-hospital services, hospitalization, surgery, nursing care, pharmaceutical services, laboratory analysis, radiology and imaging, pathological studies and analysis, therapy, rehabilitation, diet and nutrition.

• Creation, studies, analysis, updating and maintenance of clinical records.

• Invoicing and billing for services.

• Record keeping for follow-up of services.

• Lodging.

• Transportation.

 

2. Other purposes:

• Studies, records, statistics and analysis of health information.

• Maintenance of records for future service provision. Statistical and market analysis.

• Promotion and marketing of Puebla care products and services.
   

In the event that the owners do not want Puebla Care to process their personal data for the purposes set forth in paragraph b) above, they will have a period of 5 (five) days to express their refusal by sending a communication to the Department of Personal Data of Puebla Care by sending an email to the following address: turismomedicopue@gmail.com.

TRANSFER

For the provision of medical-hospital services, hospitalization, surgery, nursing care, pharmaceutical services, laboratory analysis, radiology and imaging, studies and pathological analysis, therapy, rehabilitation, diet and nutrition, creation, studies, analysis, updating and maintenance of clinical files, Puebla Care may transfer within and outside the country, the personal data in its possession to third parties subcontracted for the purposes outlined in this Privacy Notice. The third parties to which such data will be transferred are laboratories, clinics, hospitals, research centers, insurance companies, as well as any company of the same corporate group, or in its case competent authorities.

 

The consent of the owners will not be necessary when the transfers are made to companies of the same group of Puebla Care or when such transfer is necessary to provide a service or to keep the clinical files updated.

In case the owners do not wish Puebla Care to transfer their personal data, they will have a period of 5 (five) days to express their refusal by sending a communication to Puebla Care's Personal Data department by sending an email to the following address: turismomedicopue@gmail.com.

LIMITATION OF USE OF PERSONAL DATA
The owners may limit the use of their personal data by sending an email to turismomedicopue@gmail.com, indicating the limitation to the use of the desired data.

REQUEST FOR ACCESS, RECTIFICATION, CANCELLATION OR OPPOSITION OF PERSONAL DATA AND REVOCATION OF CONSENT (ARCH REQUEST)
All personal data are treated in accordance with the applicable and current legislation, therefore the holders of personal data have at all times the right to access the personal data held by Puebla Care and the details of the processing thereof, as well as to rectify them if they are inaccurate or incomplete; cancel them when they prove to be excessive or unnecessary for the purposes that justified their collection; and oppose their processing.

MEANS TO EXERCISE ARCO RIGHTS (ACCESS, RECTIFICATION, CANCELLATION AND OPPOSITION)
To initiate an ARCO Request, the holder of the personal data must submit a written request addressed to our Personal Data Department at turismomedicopue@gmail.com containing the following information:

• Holder's name

• Holder's address or e-mail address to communicate response to the request

• Documents proving identity or authorization to represent him/her in the request

• Description of the personal data on which you intend to exercise any ARCO right.

• Date of care and name of the Puebla Care member hospital where the services were received.

• Any other element that allows the location of the personal data and attention to the request.
   

MEANS TO REVOKE CONSENT

At any time the owners may request the revocation of the consent given to Puebla Care to process their personal data by sending a written request addressed to our Personal Data department at turismomedicopue@gmail.com clearly detailing the data in respect of which they are revoking their consent.

 

OPTIONS AND MEANS THAT THE CLUSTER DE TURISMO MÉDICO DE PUEBLA A.C. OFFERS TO LIMIT THE USE OR DISCLOSURE OF PERSONAL DATA

The owners of the personal data have the possibility to limit the use or disclosure of their personal data by sending a request to the following e-mail: turismomedicopue@gmail.com, indicating the desired limitation to the use of their personal data.

This Privacy Notice may be modified at any time to comply with legislative updates, jurisprudence, internal policies, new requirements for the presentation of Puebla Care services or any other cause at the sole discretion of Puebla Care. In such case, the modifications will be available on our website www.pueblacare.com , or any other means of verbal, printed or electronic communication that Puebla Care may determine for such purpose.

I declare under oath that I have read this Privacy Notice in its entirety and fully understand its scope and content. I hereby give my consent for Puebla Care to treat my personal data according to this Privacy Notice.

Likewise, I expressly consent to the treatment by Puebla Care of my sensitive personal data and the transfer of my personal data.

Likewise, I expressly consent to the processing of my information (personal data) for the "Other Purposes" described in paragraph b) of the section "PURPOSE OF THE PROCESSING OF PERSONAL DATA".
